What it does

- State machine per type — with mandatory transitions (no silent skips)
- Watcher subscriptions — teams subscribe, get pinged on state changes
- Audit log — every action recorded, exportable for SOX / SOC2
- MCP-native — query / create / update from any Claude session via the Keystone MCP server
